We provide services to the disabled and elderly to assist them with maintaining independence in the community. They are as follows:

• In-Home Services Program (Medicaid Waiver, CHOICE, SSBG and Title III)
• Nutrition (Home Delivered and Senior Center Site Meals)
• Family Caregiver Support
• Senior Employment
• Pre-Admission Screening
• Ombudsman Services
• Medicaid Select

Services under this program are provided in Jasper, Lake, Newton, Porter, Pulaski and Starke Counties.
